All new patients are requested to fill out a confidential Patient Health Record.  (This intake form is available on “documents”page so you may print and complete the form in advance to save time in the office.  Please be sure to complete both sides of the form.)

Your first visit will begin with a consultation with the doctor to discuss any health issues and your wellness goals.

Following your consultation the doctor will perform a complete Chiropractic examination including orthopedic and neurological assessments as related to chiropractic to determine if chiropractic care is appropriate.

The doctor will advise you if additional procedures such as x-ray examination or laboratory tests are necessary.  Conditions outside the scope of Chiropractic will be referred to the appropriate provider(s).

If no contraindications to chiropractic adjusting are found you will be adjusted the same day as your consultation and exam. The doctor will then give you his best recommendations for continued care and explain the three phases of care.

During the course of your care plan, re-evaluations will occur to monitor your progress and the frequency of your visits will be adjusted accordingly.
